What is Xpovio?

Xpovio is used together with bortezomib and/or Selinexo Dexamethasone to treat Multiple Myeloma in adults. It is use:

  • In combination with the medicines bortezomib and dexamethasone to treat adults with multiple myeloma who have received at least one prior treatment for their disease.
  • In combination with dexamethasone to treat adults with multiple myeloma (MM) that has come back (relapsed) or that did not respond to previous treatment Selinexo, and
    • Who have received at least 4 prior therapies, and
    • Whose disease did not respond to (refractory) to at least 2 proteasome inhibitor medicines, at least 2 immunomodulatory agents, and an anti-CD38 monoclonal antibody medicine.
  • Xpovio is also used in adults to treat certain types of diffuse large B-cell lymphoma that has come back (relapsed) or that did not respond to previous treatment (refractory) and who have received at least 2 prior therapies.
  • Xpovio was approved by the US Food and Drug Administration (FDA) on an “accelerated” basis. Selinexo in clinical studies, some people responded to this medicine, but further studies are needed.
  • Xpovio is approved based on patient Selinexo response rate. There are ongoing studies to confirm the clinical benefit of Xpovio for the following use:

    • To treat adults with certain types of diffuse large B-cell Selinexo lymphoma that has come back (relapsed) or that did not respond to previous treatment (refractory) and who have received at least 2 prior systemic therapies.

INDICATIONS

Multiple Myeloma

  • Xpovio in combination with Bortezomib and Dexamethasone is indicated for the treatment of adult patients with multiple myeloma who have received at least one prior therapy.
  • Xpovio in combination with dexamethasone is indicated for the treatment of adult patients with relapsed or refractory multiple myeloma who have received at least four prior therapies and whose disease is refractory to at least two proteasome inhibitors, at least two immunomodulatory agents, and an anti-CD38 monoclonal antibody.

Diffuse Large B-Cell Lymphoma

Xpovio is indicated for the treatment of adult patients with relapsed or refractory diffuse large B-cell lymphoma (DLBCL), not otherwise specified, Selinexo including DLBCL arising from follicular lymphoma, after at least 2 lines of systemic therapy.

This indication is approved under accelerated approval based on response rate [see Clinical Studies]. Continued approval for this indication may be contingent upon verification and description of clinical benefit in a confirmatory trial(s).

DOSAGE AND ADMINISTRATION

Recommended Dosage For Multiple Myeloma

In Combination with Bortezomib and Dexamethasone

The recommended dosage of Xpovio is 100 mg taken orally once weekly on Day 1 of each week until disease progression or unacceptable toxicity in combination with:

  • Bortezomib 1.3 mg/m2 administered subcutaneously once weekly on Day 1 of each week for 4 weeks followed by 1 week off.
  • Dexamethasone 20 mg taken orally twice weekly on Days 1 and 2 of each week.

In Combination with Dexamethasone

The recommend dosage of Xpovio is 80 mg taken orally on Days 1 and 3 of each week until disease progression or unacceptable toxicity in combination with dexamethasone 20 mg taken orally with each dose of Xpovio on Days 1 and 3 of each week.

Recommended Dosage For Diffuse Large B-Cell Lymphoma

The recommended dosage of Xpovio is 60 mg taken orally on Days 1 and 3 of each week until disease progression or unacceptable toxicity.

Recommended Monitoring For Safety

Monitor complete blood count (CBC) with differential, standard blood chemistries, body weight, nutritional status, and volume status at baseline and during treatment as clinically indicated. Monitor more frequently during the first three months of treatment. Assess the need for dosage modifications of XPOVIO for adverse reactions.

Recommended Concomitant Treatments

Advise patients to maintain adequate fluid and caloric intake throughout treatment. Consider intravenous hydration for patients at risk of dehydration.

Provide prophylactic antiemetics. Administer a 5-HT3 receptor antagonist and other anti-nausea agents prior to and during treatment with Xpovio.

Dosing Instructions
  • Instruct patients to take Xpovio exactly as prescribed.
  • Advise patients to swallow the tablet whole with water. The tablet should not broke, chewed, crushed, or divided.
  • If a patient misses a dose, advise them to take their next dose at its regularly scheduled time. If a patient vomits or misses a dose of  Xpovio, advise them to take the next dose on the next regularly scheduled day.
  • Advise patients that Xpovio comes in a child-resistant blister pack.
  • Advise patients to take their prescribed dexamethasone Selinexor price (if applicable) and prophylactic anti-nausea medications as directed.
  • Advise patients that blood tests and body weight will be monitor at baseline and Xpovio price during treatment as clinically indicated, with more frequent monitoring during the first three months of treatment.
  • Advise patients to maintain appropriate fluid and caloric intake throughout their treatment.

How should I take Xpovio?

  • Take Xpovio exactly as prescribed by your doctor. Your doctor will perform tests to make sure this medicine is the right treatment for you.
  • Follow all directions on your prescription label and read all medication guides or instruction sheets. Your doctor may occasionally change your dose.
  • Xpovio is usually take only on specific days, once or twice per week. Follow your doctor’s instructions very carefully about when and how to take Xpovio with dexamethasone and/or bortezomib.
  • Your doctor will determine how long to treat you with this medicine.
  • Take Xpovio with a full glass of water. Swallow the tablet whole and do not crush, chew, or break it.
  • If you vomit shortly after taking Xpovio, do not take another dose. Take your next dose as scheduled.
  • You may be give other medications to help prevent nausea, Selinexor price vomiting, or infections. Keep taking these medicines for as long as your doctor has prescribed.
  • You may get infections more easily, even serious or fatal infections. You will need frequent medical Xpovio price tests. You may need a transfusion if your blood platelets get too low.

Warnings

Xpovio affects your immune system. You may get infections more easily, even serious or fatal infections. Call your doctor if you have a fever, chills, flu symptoms, cough, mouth ulcers, or trouble breathing,

Before taking this medicine

To make sure Xpovio is safe for you, tell your doctor if you have:

  • Cataracts
  • Bleeding problems

Xpovio side effects

Get emergency medical help if you have signs of an allergic reaction to Xpovio: hives; difficult breathing; Xpovio cost swelling of your face, lips, tongue, or throat.

Call your doctor at once if you have:
